Working at my present job for almost a year and a half, pushing into the wheelchair department since this last June, I've found myself looking at paperwork that in no way covered all our requirements and had no uniformity in design.

The set above, which actually excludes one page, is a massive revision which covers all of our requirements, as well as presents a major overhaul of the use of space and readability.

The use of a table-type design, vs. line based, allows the paperwork to have a drastic reduction in whitespace, making the larger focus on the actual information vs. the labels of the information. When I re-created the paperwork in my first main revision, it actually seemed like I lost more space focusing on a [title] : ______________ format, as well as it made the pages dizzying, actually making it appear far more complex and, to be honest, completely disorganized.

The design I utilized here was largely focused around an example I found online, but I had some issues which I had to meet, such as the paper being writable from a distance, in the sense that the person would not be having this up in their face, but would need to be able to work on it from a standing position on a clip board while they were talking with the patient or obtaining measurements. The larger space through each of the areas allowed this to work well for writing in that situation, as well as the table format allowed for a much better workflow.

On the font end, I followed the example to an end, though I will admit I found a liking of the SlippyLight and SlippyBold fonts, which are available through T26.com [link]

Overall, a decent overhaul which met the needs of my co-worker who was missing a good amount of the information while doing evaluations, as well as a massive improvement in all the information being present in one area.

Any suggestions or ideas on improving this forward?

[I will admit, it is shrunk drastically, but this is due to this possibly falling under the line of confidential internal documentation, though it seems rather general questions which most companies in my field would ask upon patients and require to present someone with a well fit powerchair or wheelchair for their needs]
